NAFTA - Certificate of Origin Screen Fields

Exporter Name:

i. Click on down arrow to show Adresses records from Master Tables. Select a record from drop down. Address fields will be populated automatically

ii. Or enter information

State the full legal name, address (including country) and legal tax identification number of the exporter. Legal tax identification number is: in Canada, employer number or importer/exporter number assigned by Revenue Canada; in Mexico, federal taxpayer's registry number (RFC); and the United States, employer's identification number or Social Security Number.

State the full legal name, address (including country) and legal tax identification number, of the producer. If more than one producer's good is included on the Certificate, attach a list of the additional producers, including the legal name, address (including country) and legal tax identification number, cross referenced to the good described under Description of Goods.  If you wish this information to be confidential, it is acceptable to state "Available to Customs upon request". If the producer and the exporter are

the same, complete field with "SAME". If the producer is unknown, it is acceptable to state "UNKNOWN".

BOL Number/Airway Bill No: Enter BOL Number/ Airway Bill No. here

Carrier Name: Enter freight company name

Ship Date: Shipment Date

Blanket Period: Please Enter FROM and TO Dates.  "FROM" is the date upon which the Certificate becomes applicable to the good covered by the blanket Certificate (it may be prior to the date of signing this Certificate). "TO" is the date upon which the blanket period expires. The importation of a good for which preferential tariff treatment is claimed based on this Certificate must occur between these dates.

State the full legal name, address (including country) and legal tax identification number,  of the importer. If importer is not known, state "UNKNOWN", if multiple importers, state "VARIOUS

 
Order Information:

Item: Enter Item Number

Commodity Description: Provide a full description of each good. 

HS Tariff: For each good described, identify the HS tariff classification to six digits or  eight digits, using the HS tariff classification of the country into whose territory the good is imported.

Preference: Select Preference criterion (A through F).

-A: It must contain no non-North American parts or materials anywhere in the production process. This category is generally reserved for basic products such as those harvested, mined or fished in the NAFTA territory, although it can include a manufactured good with no non-NAFTA inputs.
-B: For products manufactured in the NAFTA region that have a foreign component but met the product's rule of origin requirement.
-C: For goods produced entirely in Canada, Mexico and/or the United States exclusively from materials that are already originating.
-D: In very few cases a good that has not met the rule of origin requirement can qualify for preferential NAFTA treatment if a regional value content requirement is met.
-E: This applies to certain automatic data processing goods and their parts, specified in Annex 308.1.
- F: Applies to certain agricultural goods imported into Mexico. 

Producer: State "YES" if you are the producer of the good. If you are not the producer of the good, state "NO"

Net Cost: Enter NC if the rule of origin requires a regional value content qualification and if you use the net cost method rather than the transaction value method. In ALL other instances enter NO.

Origin: Enter country of origin. Only US, CA, MX, or JNT (meaning jointly produced in Mexico and the United States) should be inputted in this block

Signatory Information: Sign and enter the name and additional information of the person filling out the form.
